About Empowerment Healthcare

Welcome to the Empowerment Healthcare website. We are a Healthcare company that provides supported living services to the disabled and the elderly. We do so by training and empowering a great pool of direct care professionals and nurses who are the core of our care model.

Empowerment has a rigorous hiring process and excellent training and support. Our Direct Care Professionals and House Managers' benefit from constant supervision, continuous and intensive training on topics that are specific and relevant to the individuals we serve.

Empowerment Healthcare currently supports numerous individuals living in a home of their choice in the Minneapolis metropolitan area. Our programs are well suited for people with physical and/or mental disabilities. We serve a number of waiver populations including but not limited to CADI, EW, BI and private pay individuals.

At Empowerment Healthcare we are committed to designing programs for individuals that strive to maintain the right balance of freedom and structure to optimize success and personal growth. Our Registered Nurses work with physicians, families, and care team members to create plans that meet physical, emotional, and social needs of individuals. Empowerment provides long-term care services in home and community based settings under our specialized services. Our services include Adult Foster Care, Assisted Living and 24-hour Customized Living. Our programs are administered by managers with deep and extensive long-term care experience in coordination with our top notch nursing staff. This model allows us to support individuals diagnosed with light to heavy medical needs such as sliding scale diabetics, tracheotomies, wounds and more.
